Please note that Sonora Quest Laboratories will not perform any testing that meets the specifications listed below without an approved prior authorization or the patient has agreed to discounted pay at time-of-service pricing if authorization is denied.
View a list of discounted tests and pricing information. Restricted testing cannot be billed to the patient’s insurance. Your office or your patient will be contacted with pricing information for these tests.
Providers who elect not to enroll with Glidian will be required to complete the prior authorization/notification process with United Health Care, Blue Cross/Blue Shield Anthem plans. The process requires providers to indicate the performing laboratory name and test name for the following services:
- Tier 1 Molecular Pathology Procedures
- Tier 2 Molecular Pathology Procedures
- Genomic Sequencing Procedures
- Multianalyte Assays with Algorithmic Analyses that include Molecular Pathology Testing
